​​Program Scope of Service and Structure

SCOPE OF PRACTICE 

Population (s) Served: 

Autumn Ridge Gardens serves adults 18 and over. With Health conditions such as, Intellectual disabilities (ID), Developmental disabilities (DD), Physically disabilities (PD), Cerebral palsy, Traumatic Brain Injured (TBI) Spinal Cord injury, Aged, Alzheimer's, Diabetes, Heart disease  Parkinson and other Comorbidities. We do not accept individuals with behaviors that create an unsafe environment for other residents, themselves or staff. We do not accept residents who are on a ventilator or have spinal cord injury above C5 and require support for breathing.

Admission Criteria 
Autumn Ridge Gardens except individuals 18 and over, who may have brain injuries, Spinal cord injuries with injuries below C-6 with complete or partial paralysis regardless of etiology or presence of other comorbidities, Dementia, Alzheimer’s, Mental Illness, Diabetes, Hypertension, Respiratory disease, Cardiac disease, Kidney disease, Post-surgical, Rehabilitative, Mobile, Non-Mobile, Wheelchair/Bed Bound Physically handclapped, Aged, and individuals with multiple other comorbidities. tube feeding, ostomies and Foley catheters. Male and female, all cultural ethnicities and belief systems. We accept Private pay, Waiver Participants and Auto insurance payers
Residents will treat other individuals in the program including employees with dignity and respect.       
Residents will be required to participate in safety drills and regulations set forth by regulatory authorities.
Residents are required to have a health appraisal completed from a medical clinician  prior to admission.
Residents are required to have a chest x-ray completed to rule out tuberculosis (TB) per state guidelines.

Settings:

Autumn Ridge Gardens provides a comfortable, safe, barrier free and therapeutic home like environment. Residents are adults 18 and over with care needs including but not limited to Activities of Daily Living (ADL’s) and Rehabilitative Services. The program has both male and female participants and staff. We offer a fully accessible environment for individuals who are ambulatory, and or wheelchair bound. 

Hours of Service:

Autumn Ridge Gardens  offers resident care 24 hours a day 7 days a week 365 day of year, with full staffing to meet the residents and other stakeholders needs. Therapy sessions are scheduled per the recommendation of the skilled therapist with target goals for therapy between 8:00 am to 5:00 pm Monday through Saturday.

Frequency of Services:
​
Therapy services are determined by the residents needs and frequency will be determined by the licensed physician and licensed therapist. Average therapy sessions can be expected to be performed 3-5 days per week at time intervals of 1-3 hours per day. Each individual service plan will vary.


​Payers and Funding Sources:

Autumn Ridge Gardens accepts reimbursements from third party payers, State and Federally funded programs and  Private funding.

Fee’s 

To be determined following assessment of the individual’s care needs. 

Referral Sources:

Patients may be referred for services by local physicians including primary, orthopedic, neurologist, licensed therapist, community resources, word of mouth from previously person served and stakeholders.

Activity Limitations

Autumn Ridge Gardens  serves individuals with many levels of care needs including individuals that require maximum assistance which will be determined at assessment. 

​Cultural Needs

Autumn Ridge Gardens will do our best to ensure your needs are met and that belief system is respected and maintained. Our staff does accommodate a variety of food types according to cultural preference and special medical diets. We respect and honor individuals’ worship and language and accommodate whenever possible to embrace others' cultural beliefs and practices. 

Intensity of service

​Autumn Ridge Gardens has many resources and abilities to fulfill the needs of the  persons we served and our stakeholders. If this is not possible, we will provide residents and stakeholders with resources to help provide services to meet their needs.

Intended discharge/Transition environments.

It is the intention of Autumn Ridge Gardens  to partner with our residents, stakeholders and employees to meet goals and to provide the best possible outcome. Autumn Ridge Gardens can be your long-term care partner if the option of returning to your formal living arrangements is not an option.

​Estimated length of stay

The estimated length of stay can range from a few weeks or days to permanent placement. The length of stay will be determined through ongoing assessment, feedback and outcome measurements of each person served.

RESIDENT RIGHTS:
(a). All residents receiving services from Autumn Ridge Gardens shall retain all rights, benefits and privileges guaranteed by Federal, State, and local law, except those specifically lost through the due process of law.
(b) Residents have the right to live in the community of their choice without restraints on their independence, except those restraints to which all citizens are subject.
(c). Residents have the right to be treated with courtesy and dignity and are at all times entitled to respect for their individuality, and the recognition that their strengths, abilities, needs, and preferences are not determinable on the basis of a psychiatric diagnosis.
(d) Residents have the right to be notified of all rights accorded them as recipients of services at time of admission or intake, and in terms that he or she understands.
(e) Residents have the right to be treated in the least restrictive setting to meet their needs. Use of physical restraint will be limited to emergency situations where there is a reasonable expectation that the situation would result in serious injury or death to the consumer or other persons in the environment. 
(f) Residents have the right to receive services conducted in a manner reflecting quality professional and ethical standards of practice and shall be apprised of the organization’s code of ethics/conduct.
(g) Residents have the right to receive services without discrimination based on race, color, sex, sexual orientation, age, religion, national origin, domestic/marital status, political affiliation or opinion, veteran’s status, physical/mental handicap or ability to pay for services.
(h) Residents have the right to be treated in an environment free from physical abuse, sexual abuse, physical punishment, or psychological abuse by threatening, intimidating, harassing, or humiliating actions on the part of staff.
(i) Residents have the right to be fully informed of the services to be provided, the right to consent to services, and the right to refuse services (with the exception of legally mandated services) without fear of retribution or loss of rights.
(j)Residents have the right to privacy during facility visits.  Individuals and/or group visits are permitted only when the purpose of the visitation is education or professional in nature.  Planning for outside visitors shall provide for limited interruption of consumer routine, therapeutic or rehabilitative programs, and related activities.  Persons served will be given notice of such visitation.
(k)Residents have the right to confidentiality.  Information may not be released without the consumer’s written permission, except as the law permits or requires.  
(l)Residents, or the consumer’s legal guardians, have the right to review the consumer’s record at any reasonable time upon request, including prior to an authorized release, and shall be afforded the assistance of an appropriate clinical employee in cases where a reasonable concern exists of a possible harmful effect to the consumer through the misinterpretation of information in the record.
